What is the SLMC Identity Card?

The Sri Lanka Medical Council (SLMC) Identity Card is the official identification issued to registered medical & dental practitioners. It serves as proof of SLMC registration and may be required when collecting certificates, documents, or accessing services provided by the Council.

Who can apply?

The following persons are eligible to apply for an SLMC Identity Card:

1. Medical & Dental practitioners currently registered with the Sri Lanka Medical Council.
2. This is usually issued together with the first registration. However, those who did not obtain their ID card at the time of registration may apply for their first ID card through this service.
3. Registrants requesting a replacement identity card due to loss, damage, or expiry of their existing card.

Application Fee

The prescribed fee for an SLMC Identity Card is Rs. 760.00. Payment should be made online to the Sri Lanka Medical Council as instructed by the SLMC. Please ensure that your SLMC Registration Number and the relevant payment category displayed on the payment page are correct before making the payment.

Required Documents

Please upload the following documents together with your online application.

• Colour photograph as per the Specifications given by the SLMC
• If applying for a replacement due to loss, attach a scanned copy of the Police Entry Report.
• If replacing a damaged SLMC Identity Card, return the existing damaged card when collecting the new card.

Application Process

• First-time applications or replacements due to loss: The new SLMC Identity Card will be posted to the applicant or may be collected in person, provided that the online application and payment are completed correctly.
• Replacements due to damage or requests to add additional information: The application and payment must be completed online. The existing identity card must be handed over when collecting the new card. These identity cards will not be sent by post.

Processing Time

The processing time for an SLMC Identity Card is approximately 7 working days. Applicants will be notified by SMS and email once the card is ready for collection.

Collection of the Identity Card

Applicants are encouraged to collect their Identity Card personally from the SLMC office.

If personal collection is not possible, the Identity Card may be collected by a nominee on behalf of the applicant. The nominee must present a duly completed Nominee Form, which can be downloaded from here.
